WAIVER: I hereby release Cherie Maitland and Our Furry Friends from any liability
associated with the transporting of my dog(s) as well as boarding/training of my dog at her
home in Pioneer, and Training Centers in Placerville and Pine Grove, California. This release of
liability includes risk of injury to my dog, myself and my family, other people and animals,
exposure to contagious disease, and loss, escape or theft of my dog. I understand that animal
training is an inherently hazardous activity, and that I am exposing myself, my family and my
dog to the possibility of a bite or injury by my own or another dog. I certify that my dog is not
vicious or aggressive. I have read and agree to Refund/Cancellation & Late Appointment Fees
Policy.

Refund/Cancellation & Late Appointment Fees Policy:

  • If you change your dates there will be an administrative fee of $15.
  • If you cancel dates more than 2 weeks ahead, you will receive your deposit back minus 1
    day of the highest service you had reserved.
  • Less than 2 weeks and more than 1 week, you forfeit 2 days worth of the highest service.
  • If you give less than 1 week and more than 48 hours notice, you forfeit 3 days of the highest
    service reserved.
  • Less than 48 hours, you forfeit 4 days of the highest service reserved for holding your dates.
  • If you are more than 30 minutes late to a designated pick-up or drop-off time, you may be
    charged at $25/hour. Please allow plenty of travel time for delayed flights etc.
  • If you call before the designated time with a new time or add on a day of boarding if I have
    the space, there will be a fee of $15 plus the extra boarding at full price. If I am unable to
    meet you on your new day, i.e. I am closed Sundays and Holidays, you will be charged for
    the additional days at full price.
  • If you come home early, there will not be any refunds, unless you give me a week's notice.

Since I am a small, cozy operation, I only take in 1 dog at a time, so sudden last minute changes
may mean that I have turned away other clients. I hope you understand.

After you have contacted me about your dog and your required dates etc, please print form, completely
fill out, sign and mail to the above address with your deposit check of 50% of the total, or minimum of
$100 - whichever is more, and copies of the last Rabies and Bordetella vaccines, and the last 2 of
the DHLPP vaccines.

Keep the ITEMS TO BRING LIST FOR YOUR PACKING. Your dates are finalized once I receive your
deposit check and the above list of forms and records. Balance is due when you drop off your dog.
